About Us

At Ascend, we partner with designated agencies to connect families and providers with skilled Direct Support Professionals (DSPs) who deliver Home and Community-Based Services (HCBS). Our role is to ensure that designated agencies can consistently meet the individualized needs of their clients by providing qualified and compassionate staff.

About Us

Our Role in Community Education:

Ascend is dedicated to raising awareness about Medicaid-funded services in NYC, educating schools, social workers, healthcare providers, and other community members to expand access for Medicaid recipients and families.

Services (HCBS) can significantly benefit children with disabilities,

surroundings, such as at home or in the community. We offer support through one-on-one or group

Teens & Young Adults
Home and Community-Based Services (HCBS) offers essential
support to pre-teens, teens, and
young adults through tailored interventions that address mental
health, emotional regulation, and social skills. These services help build
self-esteem, foster healthy peer relationships, and promote positive
lifestyles. HCBS also provides crisis
intervention, life skills training, and vocation assistance aiding in, employment readiness and financial independence. Overall, C B S empowers young individuals to navigate their developmental stages, overcome the challenges of
adolescence, achieve their goals, and thrive.
